GUARDIANSHIP SERVICES
- Preliminary functional assessment of the ward's ADLs (Activities of Daily Living) and IADLs (Instrumental Activities of Daily Living)
- Determine and monitor residence
- Consent to and monitor medical treatment
- Consent to and monitor non-medical services such as education or counseling
- Release of confidential information
- Make end-of-life decisions
- Maximize independence in least restrictive manner
- Report to the court about the guardianship status at least annually
- Advocate on behalf of the ward for housing and living arrangements; employment and training; entitlements and benefits; basic civil and property rights; religious rights; uncensored communication. medical rights; and parental rights
CONSERVATOR/FIDUCIARY SERVICES
- Identify all income, assets, liabilities
- Establish a balanced monthly cash flow
- Pay all monthly obligations
- Negotiate with all creditors
- Review all investments for maximum benefit and safety of principle
- Ensure all property is under adequate insurance
- Safeguard all assets for the benefit and use of the Protected Person
- Liquidate assets when necessary
FEES
- Initial Contact Through Court Appointment as Guardian of Person or Fiduciary:
$125 hourly plus out-of-pocket cost
- Guardian/Fiduciary Case Office Set-up Fee:
Minimum of $500 plus out-of-pocket costs
- Guardianship Fee: $250 minimum per month
- Conservatorship Fee: $150 minimum per month
- Probate Court Appointed Visitor Reports:
$125 hourly, plus out-of-pcoket costs, inclusive of mileage unless otherwise noted. Includes written report with copies sent to all interested parties
- Court Testimony:
$150 per hour plus out-of-pocket costs
- Care Managment Services/Private Non-Medicical Care Attendants:
